Nutrition in Latin America & The Caribbean

Nutritional health can impact all ages throughout the many cultures around the world.

The nutritional information about children health is limited in Antigua and Barbuda, but the information available suggest the level of under-nutrition, for children less than 1 year of age is remains constant at under 5% of the population. In contrast, the level of overweight children is on the rise. A 1993 Ministry of Health survey found obesity, with a BMI: > 30, is around 5%, where the majority of the cases were men. In the Bahamas female adolescents show a higher tendency to obesity in comparison to men. Among adults, (15-64 years), a fifth of the population suffer from obesity. Obesity negatively impacts the health and height of a countries population. Columbian women between 15 – 49 years have a 31-41% % of having an overweight BMI level.

As adults get older, the risk and rate of obesity rises. In adults over 65 years of age, a quarter of females and a sixth of men are overweight. A 2000 study in Chile, shows the elderly in Santiago as being ~12% underweight, ~22% overweight and ~26% obese. The BMI in Cuba has roughly 9% of women and 6% of men as chronically energy deficient.

DIET

The most common nutrient deficiency is iron deficient anemia; where up to half the children from ages 1-4 are afflicted. In the Bahamas and Venezuela, pregnant women are at risk to be anemic. Antigua and Barbuda, have a national diet of cereal, vegetables and fruits that have not been fortified with iron. Cuba has a national plan to fight against anemia by education the public about iron supplementation and standardizing the fortification of food with iron. The traditional Cuban diet consists of rice, beans, chicken meat or pig meat, bananas, yuca, sugar and coffee.

In the Bahamas, there has been seen a trend towards the increased consumption of animal fatty foods, sugar and salt products. The population also suffers from a decreased level of exercises, and not enough fruits and vegetables are being eaten. In Barbados, there has been seen an increased reliance on animal protein as a main source of diet. This causes an increase in the amount of fat an average person consumes. This can lead to a rising obesity in the country. Chile also shows an increase consumption of meat, milk, high saturated fat and energy foods. This leaves less room for important optimal height nutrition such as fruits, vegetables, cereals and legumes. Almost half of the Costa Rica population has critical iron deficiencies, where there is an inadequate consumption of the countries sources of iron: beans, wheat products and meat. The main source of protein is meat, fluid milk, cheese and rice, about 11% of the total energy. Carbohydrates are consumed regularly, but the fat levels are higher than recommended.

Conversely, Vitamin A levels are largely standard in some areas such as Antigua and Barbuda but are a problem in places such as urban centers in Argentina and for women in Bolivia. Extreme cases of Vitamin A deficiency lead to cases of night blindness. Brazil faces Vitamin A deficiency, strongly affecting children under five and pregnant women. Thus, the resulting children born also face this deficiency, with a percentage as high as 56%.

Steps such as the iodization of salt since 1967 are a positive move to prevent iodine-deficiency disorders. Overall, the prevalence of goiter has gone down, in places like Bolivia where students of 18 years have seen 40% less cases of the nutritional disease. In Brazil, government action made effective by monitoring the iodization of the salt industry has greatly reduced this deficiency.

Breast-feeding is instrumental in infant and mother health. In Belize, breast-feeding is more common in rural areas rather than urban cities. To combat this trend, a young child-feeding program is available to teach mothers helpful health practices.

Sometimes countries are unable to grow their own food. This results in a reliance of food aid from other countries. A country like Bolivia has relied on external help to develop not only healthy nutrition, but also basic services and infrastructure to support the population.

ECONOMICS OF FOOD

Food is generally available in this region. The Bahamas is a diverse area, where the average income varies greatly between the different islands. When a household is at economic risk with a low income, it leads to insufficient food intake. The unemployed and illegal immigrants are especially at risk, for not achieving their optimal health and height due to living in poverty. In Barbados, the average amount of money spent on food dropped drastically from 51% to 40% in the twelve years between 1969 and 1981. In as early as 1999, the World Bank identified roughly 8% of the population as living below the poverty line. In Guatemala more than a quarter of the people have an income less than US$1, so about 2.8 million of the 1999 population are living in poverty. Poverty extends to water, as only 67% of Guatemala homes had access to piped water. In Panama poverty is unequal, with the indigenous population groups being the poorest.

Poverty is a major factor in a country’s health and height possibilities. A country like Belize is vulnerable, to economy as one forth of the households are below the poverty line.

With institutions such as Chile’s Social Food Programs, there is better food access for more sections of the population, despite economic status. In Columbia, the country’s economic improvement over 40 years has improved the social conditions.

HEIGHT, DISEASE, DEFICIENCY

In Argentina, anemic is high among women that are of childbearing age. This lack of an important micronutrient, that is essential for proper growth contributes to the malnutrition of children, so that they are not able to reach their proper height. There are global statistical bodies that measure height and use their data to judge health standards. The Census of Stature, looked at Bolivian student in 1990 and confirm that men faced a higher level of stinting, where the problem is more prevalent in rural areas, such as the provinces of Potosí, Cochabamba and Chuquisaca.

In Argentina, STUNTING is common and affects 5.3-19.7% of the population, varied by regional geography. In the Bahamas, a report from the Ministry of Health in 1995 show that children 4-9 years old are suffering from various levels of improper nutrition. Children range from being underweight (~7%), stunting (~13%) and even wasting (~6%). Wasting is a step further than stunting, where muscle and fat tissue are severely depleted. This can be caused by acute malnutrition and disease. In Ecuador, a 1986 survey found stunting to have gone down from 34% - 26%.

In Guatemala, a little less than half of children are stunted. . In Mexico, nearly 1.8 million children less than five years are stunted, approximately 800 000 children are underweight and 213 000 children are wasted. What this means, is that the growth retardation continues, as they get older affecting both men and females.

Height is compromised by poor health.

The World Health Organization identified the area of Toledo in Belize where stunting is common (39%) in infants. In Brazil, the regions of Maranhão and Piauí have the highest statistic of underweight children, where stunting is a major problem. Wasting is not widespread, not making it a country wide epidemic. In Costa Rica, women between 15 and 19 years old face either the problem of being underweight or obese.

Countries that focus their resources on health are able to work to improve these conditions. In Bolivia, the condition of children below 5, has improved where children grow taller and do not suffer from stunting of being underweight. In a 2000 survey in Cuba, malnutrition of children went below 5%, putting the country at front of other developing countries.

ENVIRONMENT

Height, and nutritional health depends comes from a countries economic factors. In Argentina, the level of obesity rises in cities such as Santa Cruz and Chubut. Further, a statistic study on 18 years old males found that the average BMI level was 22.4 kg/m2, which is within the standard levels. That being said, height for men varies widely between geographical regions in Argentina. The lowest average height is found in Jujuy (167.5 cm) and San Luis (167.7 cm), while the national average is higher in Buenos Aires, Rio Negro and Cordoba. In the Bahamas, children health also varies by region. High levels of malnutrition were found on the Crooked Islands, the Family Islands and Acklins.

In Guatemala environmental droughts and floods, deforestation and soil erosion lead to decreased food production. Thus, Guatemala relies on imports from other countries, focusing on cereals, milk and animal fats.

In Belize, there are a different ethnic groups and classes that live in the same country. The difference in cultures is a difference in the type of health patterns that can be seen from the population. In Brazil, the majority of the land is uniform, aside from the Amazon and arid northeast area of Sertão, so the food items are very similar across the country.

Different economic environment vary with their rates of poverty and illiteracy, infrastructure and basic services. The environment also dictates how much possibility is there for farming land and food production. Uruguay national programs about nutritional health practices should be mandated to the entire population. In Chile, a concentrated effort into health issues has led to improved access to water, better sewage systems and education. A negative side effect to this industrialization is the increase in less healthy lifestyles in the cities. This leads to the opposite effect: diseases caused by over-nutrition such as cardiovascular diseases and certain cancers.

This points to the fact that environmental factors should as nutrition and area can actively influence height.

In Mexico, there are major regional differences. Where a person lives influences their health. Generally malnutrition is focused in the south and rural areas, while obesity occurs more often in the north and urban areas.

Nutrition In Africa

Different parts of Africa face different nutritional challenges but under-nutrition is predominant.

There is an influx of energy dense food such as oil and sweetener that paradoxically is causing the emergence of obesity health problems. This is more common in the urban part of the country. The huge economic disparity between the rural and urban centers creates very variable living standards. Health infrastructure is being built, but specialized medicine has not spread to outer towns. Most urban places are entering a transitional period where there is massive upheaval in the type of food eaten and the change to physical activity. This can partly be attributed to globalization and the exposure to the different unhealthy food options available: street foods, snacks and sugar-rich food.

The major nutrient deficiencies come from micronutrients. Vitamin A deficiency is a major trend, where supplementation and education campaigns are directed at young children and pregnant women. Unfortunately, the lack of a unified and far reaching health care system is causing the re-emergence of tuberculosis and diarrheal diseases. Goiter is something that is commonly seen. It is the swelling of the neck from a swollen thyroid gland mostly likely stemming from iodine deficiency. Thus the regulation of salt iodization is needed. Countries like Guinea implement a salt iodization program to combat goiter with various degrees of effectiveness.

Anemia is another health condition from iron deficiency that affects most rural women. Many women also suffer from chronic energy deficiency (low BMI).

Some solutions to these nutrition issues are to introduce Vitamin A fortified cotton oil and wheat flour with iron, zinc, folic acid and B vitamins. The mixture of the natural combination of supplementation and food is very helpful in raising nutritional levels.

The lack of nutrition is negatively affecting height. Upwards of 20% of the children are stunted due to malnutrition where poverty is a major hindrance to getting access to health care. In the country Benin, in 2008 about 40% of children under 5 were stunted. In Chad, 41% of children under five were affected by stunting. On the other end of the spectrum, in 2006 round 9% of children were overweight under five years.

Notably, there has been a trend to bottle-feed rather than breastfeed infants, which can lead to improper nutrition practices. Teaching mothers to exclusively breast-feed has slowly improved in countries like Ghana but needs to be sustained.

Proper education and supplement interventions are needed to change social practices and promote better health. In Ethiopia, vitamin A supplementation and long-term food-based interventions are desperately needed. Vaccination coverage also contributes to an overall decline in child mortality.

The diet of Africa is not the same and differs depending on the resources of the country. The diet Benin heavily favors starchy roots such as yam and various cereals. The Cape Verdean diet centers on cereals (maize, rice, and wheat), vegetables, starchy root and fish. Morocco has agriculture possibility, with the production of wheat and barley.

Closer to the coastline, fish are staples in the diet. Rice is become more and more prevalent here and around the world as it provides more than one fifth of the calories consumed. The advantage of rice is its ability to adapt and grow in various terrains around the world.

The demographics of countries such as Chad are constantly changing. Political turmoil and civil unrest cause major movement of people and can have a large number of displaced refugees coming in. In Mozambique a post independence civil war from 1977 to 1992 devastated the country. This majorly regressed agricultural production by destroying buildings and families. Yet, since then the country is on it’s way to a period of recovery.

Possibilities for wealth might come in the form of looking for alternative natural resources. Oil is a relatively new additional to Chad’s economy. Zambia's economy was the copper mining industry but recent improvements in agriculture have been beneficial.

Water is extremely important to maximum health and growth. Countries like Chad still face water sanitation challenges which negatively impact the growth of its people.

Countries such as Algeria depend heavily on food imports, and the diversity of the food index is still something that needs to be improved. In places such as Benin, the majority of livestock is dependent on imports and thus the supply is limited and not spread equally across the country. This is also true for places such as Cape Verde are restricted to the small portion of the land that can be farmed and inconsistent rainfall. The land is a major for nutrition as a country like Kenya has been faced by both drought and flood. The over-exploitation of resources has also damaged the natural environment. Both in the 1990s and in 2001-2002, Malawi was similarly affected by drought that created widespread famine.

Diseases such as malaria and HIV/AIDS epidemic lower life expectancy and increase mortality rates particularly for children. In Kenya, the rate of immunizations has slowed and needs to be improved.

Having a unified national policy on food and nutrition and education about the benefits of having a diversified diet, opens opportunities to improve the nutrition of the population.

ENVIRONMENT

The land available for food production is important. Bahrain has harsh environmental conditions, so that agriculture is small part of the countries food. A way to combat environmental factors is improved irrigation, and techniques to improve soil fertility.

The government is able to positively change health standards. In Bahrain, medical care is provided free to its people, leading to an improvement in life expectancy. In Kuwait education, health care and immunization is free to the population.

In both Jordan and Palestine, the terrain is a dry, making water a valued and limited resource. The lack of water means less agriculture can be produced so that the country is heavily dependent of their imports of food. The global food market is facing increasing prices, so that import dependency becomes difficult to sustain.

Environmental factors directly impact a person’s health and living conditions. In Lebanon, civil conflict halted social development from the mid-seventies to the beginning of the nineties. This is more strongly seen in Palestine, with the start of a second Intifada in September 2000 that threatens school enrollment, child mortality, and maternal mortality. Political unrest has damaged and regressed the country, so that internal development has stalled for humanitarian relief. Conflict in Sudan, along with climate problems such as floods cause food shortages and population displacements.

DIET

Bottle-feeding is very common in Bahrain and there have not been enough steps to educate women about the importance of continued breast-feeding.

Iron deficiency is a central problem, affecting many young children and women. In Iran, parasitic and infectious diseases, poor weaning practices, gastrointestinal bleeding, and short birth intervals contribute to anemia in women. Anemia affects almost a quarter of young children and half of women of childbearing age. Vitamin A and iodine deficiency differ widely in this area of the world. In Bahrain they are not considered public health problems but in Egypt the levels are insufficient along with iron, calcium, selenium and zinc. In Egypt, the rate of iodine deficiency produced goiters is higher than 5%, but that is combated with supplementation and universal salt iodization programs. Lebanon also sees deficiencies in vitamin D, folate and vitamin B12.

Cereal is the main staple in the Egyptian and Iranian diet, but diet varies on population distribution: urban areas consume more animal products. In Iran, the Province of Tehran has highest fat intake with their main food source coming from animal products. In Jordan, there has been a positive trend to diversify the diet with a variety of wheat, rice, vegetables, and animal products such as dairy products and meat.

ECONOMICS OF FOOD

In Egypt, there is becoming a much more varied food sources available, but they are distributed unequally among the various social classes. Living in poverty in Iran, means not having enough access to food, health services, and a lack of nutritional awareness. In Sudan more than 90% of the population suffer from poverty and food insecurity.

The economy of the country Kuwait is fueled by petroleum instead of agriculture, so that most of the food needs to imported from the outside. The fish industry is another resource, and contributes to the countries exports.

HEIGHT, DISEASE, DEFICIENCY

There are two main problems for child health: STUNTING and obesity. Both health conditions directly impact future adult health. In Iran, roughly a fifth of children less than five years of age are stunting, and roughly 7% wasting. In Palestine, one out of ten preschool children are stunted, especially in the Gaza Strip.

In Tehran, the stigma of body issues contributes to girls under-eating for fear of gaining weight. On the other spectrum, around 13% of women in Tehran are obese.

Living in a village in Iran means that a person is more likely to have malnutrition, especially in the south, southeast end, compared to the north. Inadequate diet slows down growth. Many children get non-milk foods very late, so that their rate of growth slows down considerably when compared to standard height gain in normal children.

On the other hand, rapid industrialization has drastically changed eating patterns. Eating out, and not doing enough exercise is common so that adults become overweight and obese. This is true in Lebanon and particularly evident in Kuwait, where a whopping 80% of the adult women are overweight or obese. The country’s discovery of oil considerably changed the type of food that the people eat: traditional food is replaced by fast food. This over-nutrition leads to diabetes, hypertension, cardiovascular diseases and cancer. The early onset of such problems can be seen among school children. Both malnutrition and over-nutrition impact health and height.

Nutrition in Asia and Southwest Pacific.

This area is both culturally and geographically diverse. Malnutrition is very common, with countries such as Bangladesh having one of the highest percentages. To give a clear idea, about 9.5 million children are stunted or not fully developed in their height. In Bhutan, stunting occurs in 56.1% of the children. In Cambodia, malnutrition of children is a major problem. In China, the highest level of stunting was seen in the same provinces as those that were underweight. Healthy weight helps maintain height.

It is increased education and implementing of nutrition programs that are working to keep these levels down. Nutrition and height are closely connected.

Deficiency of Vitamin A, iron, iodine and zinc are common. Bangladesh has children who are anemic and women who are chronically underweight in their BMI. Nutrition not only ensures proper development but is also linked to cognitive abilities and protection against disease. In Cambodia, goitre affected 12% of school children from 8 to 12 years old. Protein-energy malnutrition (PEM) is a problem in the Philippines.

In opposition, heavily dense cities like Beijing have the problem of obesity. The last decade has seen an opposite movement of BMI levels where they now are above normal. This has caused a rise in diseases of the heart and the vascular system seen in places like the Philippines.

For women’s health, anemia is a serious complication of pregnancy in Bhutan. Other complications of nutrient deficiency are night-blindness, conjunctival xerosis and 0.1% with corneal ulcer with keratomalacia. Other diseases include Acute Respiratory Infections (15%), Intestinal Infections (10.6%) and skin diseases (10%) in Bhutan children. In Lao, medical facilities and immunization is low resulting in high child and maternal deaths.

Most mothers breast-feed their children but giving complementary food too early is a problem. Some infants receive rice in their first month of life, causing digestion complications. Increased education programs see this practice declining.

Food sources vary across different countries but food production is faced with the challenge of keeping up with rapid population growth. Countries like Bangladesh focus on cereal production but have a limited area of land to use for agriculture.

Research on non-cereal crops supported by government is important to diversify the diet to include livestock and fruits/vegetables. A lack of access and utilization of food is problematic.

Similar to Africa, there are large differences between the nutrition of urban and rural areas, where double the energy intake can be seen in the cities (1500 kcal/day versus 3000 kcal/day). A country like China is huge and each province has its own nutrition and health standards. A country like Fiji is rapidly moving towards industrialization causing the gap between under-nutrition and over-nutrition to increase.

The province of Sri Lanka has the highest population and the best health facilities and must focus on expanding that expertise to all areas of the country.

Other roadblocks to a healthy population are political and civil turmoil. Cambodia’s terrible 20-year unrest has made it hard to collect data but recently more efforts have been made to increase development. Poverty is a major factor as places like in Fiji, a quarter of the population live under the poverty line. Lao People's Democratic Republic is one of the poorest countries in the East Asian region with an estimated per capita income of US$400 in 1997.

The problem is not only availability, but also access and utilization of food, as well as a lack of diversity in the diet of the typical Cambodian.

Access to safe water and sanitation at the local level is a key step to normal growth.

In China the majority of the diet comes from cereal, rice and wheat flour providing the main source of energy ranging between 54% to 78% respectively in Beijing and Gansu. In Fiji there is a large reliance on food imports. The diet has shifted from traditional root crops to more processed imported food, animal fat and cereal. The Lao diet usually has three meals are consumed per day of rice with small portions of vegetables, and fish.
